29 CFR 1910.333(c)(3)(i)(A)(1): When an unqualified person was working in an elevated position near overhead lines with voltages to ground rated at 50 kV or below, the location was not such that the person and the longest conductive object he or she may contact could come closer than 10 feet to any unguarded, energized overhead line. Site- On or about November 12, 2024, an unqualified employee using a SST40EIH-02 Versalift aerial device to lift an energized 240 volt triplex powerline, communication lines, and a conductive messenger line with the bucket was not protected from electric shock and burns where the employee and conductive utility lines were brought within 10 feet of an energized 34.5kv overhead powerline. When employee and the conductive utility lines encroached the minimum approach distance for an unqualified person, multiple arch flashes occurred where the employee sustained serious injuries.
