29 CFR 1910.333(b)(2)(iv)(B): Before circuits or equipment was worked as deenergized, a qualified person did not use test equipment to test the circuit elements and electrical parts of equipment to which employees would be exposed and did not verify that the circuit elements and equipment parts were deenergized: a) The Young Men's Christian Association of Greater Toledo dba YMCA and JCC of Greater Toledo - Sylvania, Ohio: On or about January 24, 2012, a qualified person did not use test equipment to verify deenergization of circuits before employees replaced a ballast for a florescent light fixture located in handball court #2. While replacing a ballast, an employee received electrical shock injuries from the 277 volt lighting circuit(s). In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
