29 CFR 1926.1204(d)(2): Employer did not provide ventilating equipment needed to obtain acceptable entry conditions: On or about July 8, 2024, at job site located at 9220 Old Santa Fe Road, in Kansas City, MO, the employer failed to protect employees from asphyxiation hazards associated with permit required confined spaces. An employee was suffered fatal injuries when the employer did not provide ventilating equipment needed to obtain acceptable entry conditions.
