Text For Citation: 01 Item/Group: 001E Hazard: LOCKOUT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to; fire, explosion and steam hazards. (a) Boiler Room; The three "Titusville" automatically fired natural gas boilers were being operated with a "Wagner" Lockout Box and the "Wagner" Lockout Box on #1 "Titusville" boiler was not functioning in the manner in which it was designed. These "Wagner" Lockout boxes were not approved by the boiler manufacturer nor by a nationally recognized testing agency. Among one feasible and acceptable method of abatement would be to have a nationally recognized testing agency accept the device for it's intended service in accordance with the manufacturer's guidelines and the code requirements of the American Society of Mechanical Engineers CSD-1-1995. ABATEMENT DOCUMENTATION IS REQUIRED FOR THIS ITEM
