29 CFR 1926.1203(e)(2)(v)(B): The forced air ventilation was not directed as to ventilate the immediate areas where an employee was or will be present within the space and must continue until all employees have left the space: (a)PKG Contracting, Inc. at 29698 135th Street, Selby, SD 57472: On or about and at times prior to February 24, 2025, the employer did not ensure that the forced air ventilation properly ventilated the immediate areas where employees were tasked to work in a confined space. This exposed employees to carbon monoxide poisoning.
