29 CFR 1910.178(o)(1): Load(s) on powered industrial truck(s) were not stable or safely arranged: Warehouse- On or about July 8, an employee operating a Raymond 740 R35TT reach truck sustained an injury when they manually adjusted the position of the forks on the back rest and did not properly secure one of the forks. One of the forks dislodged from the back rest and dropped a pallet of corrugated cardboard onto the reach truck operator, who dismounted the truck, and was beneath the loaded forks.
