29 CFR 1926.1204(c)(4): Purging, inerting, flushing, or ventilating the permit space as necessary to eliminate or control atmospheric hazards; On or about July 8, 2024, at job site located at 9220 Old Santa Fe Road, in Kansas City, MO, the employer failed to implement procedures to eliminate or control atmospheric hazards, including hydrogen sulfide, in an active sewer system. An employee suffered fatal injuries when overcome by hydrogen sulfide during an active sewer system entry to perform system tie-in and installation activities.
