29 CFR 1910.146(k)(3)(ii): Each authorized entrant did not ensure the other end of the retrieval line was attached to a mechanical device or fixed point outside the permit space in such a manner that rescue could begin as soon as the rescuer is aware that rescue is necessary. On or about July 1, 2024, at the work site where authorized maintenance employees entered the RBI/ Pit (Permit defined Confined Space) without a retrieval line attached to a mechanical device or fixed point outside the space for emergency retrieval.
