29 CFR 1910.134(k): The employer did not provide effective training to employees who are required to use respirators a) Washington Operating, LLC dba Premier Washington Rehabilitation and Nursing Center located at 36 Old Hickory Ridge Rd, Washington, PA 15301: On or about November 9, 2021, employees were not provided with effective training who are required to use respirators. Employees are required to wear an N-95 respirator when they have exposure to a person or persons with suspected or confirmed COVID-19.
