29 CFR 1928.57(a)(11): Where a moving machinery part presented a hazard during servicing or maintenance, the engine was not stopped, the power source was not disconnected, and all machine movement was not stopped before servicing or maintenance was performed, except where the Employer could establish the following in 1928.57(a)(11)(i-iii): a) Facility: On October 2, 2018 and at times prior thereto the mechanics helpers were not protected by their own personal lockout device when performing scheduled or unscheduled maintenance activities along with the head mechanics. This included but may not be limited to scheduled and unscheduled maintenance on the grizzly, picker or chopper machines. Note) Certification of corrective action is required for this item.
