29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: (a) On or about November 14, 2016 the employer did not ensure that while being cleaned the foil press machines were turned off in accordance with the establish written procedures, exposing foil press operators to hazards associated with the unexpected release of hazardous energy; (b) On or about February 15, 2017 and at times prior thereto, the employer did not ensure that while being repaired/maintained the foil press machines were turned off in accordance with the establish written procedures, exposing mechanics to hazards associated with the unexpected release of hazardous energy.
