29 CFR 1910.134(m)(2)(i): The employer did not establish a record of the qualitative and quantitative fit tests administered to an employee including all of the information in section 1910.134(m)(2)(i)(A) through 1910.134(m)(2)(i)(E): (a) Piezo Kinetics, Inc. - The employer's fit test records did not include the type of fit test performed, and the specific make and model of the respirator tested, on or about August 29, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
