29 CFR 1910.147(d)(5)(i): The employer did not ensure that all potentially hazardous stored or residual energy was relieved, disconnected, restrained, and otherwise rendered safe. On April 17, 2013 or at times prior thereto at E-1 exchangers, in the Catalytic Hydrodesulfizer Unit 2 (CHD-2) at the ExxonMobil Beaumont Refinery the employer did not ensure that procedures utilized to de-inventory, and neutralize the equipment during turnaround were complete and effective to control the unexpected release of residual energy.
