29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): (a) Beaver Builders and Excavation, Inc. @ 4251 Grove St., Denver, CO 80211: On 1/16/13, the employer did not ensure that an employee was protected from cave-ins by a protective system. An employee conducted residential sewer tap repair while inside a trench, which measured approximately 8 ft. x 3 ft. x 8 ft. deep. This condition exposed an employee to struck-by hazards.
