29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. Location: 198 Tecumseh St. Fall River MA On or about March 5th 2005, employees were exposed to falls to the ground from heights up to 24 feet to the ground while working on a pump jack scaffold system where triangular braces were not used at the bottom of scaffold poles.
