29 CFR 1926.1203(e)(2)(v)(A): 29 CFR 1926.1203(e)(2)(v)(A): Continuous forced air ventilation was not used when an employee entered a space and until a hazardous atmosphere had been eliminated: (a)PKG Contracting, Inc. at 29698 135th Street, Selby, SD 57472: On or about and at times prior to February 24, 2025, the employer did not ensure that continuous forced air ventilation was used to entirely eliminate a hazardous atmosphere before employees were tasked to work in a confined space. This exposed employees to carbon monoxide poisoning.
