29 CFR 1910.23(d)(12)(i): The employer did not ensure that the step-across distance from the centerline of the rungs or steps for through ladders is not less than 7 inches (18 cm) and not more than 12 inches (30 cm) to the nearest edge of the structure, building, or equipment accessed from the ladders. On or about October 22, 2024, and at times prior thereto, in the Plant Two Production Station 16 catwalk; catwalk guardrail rail systems being utilized as fixed ladders had a step-across distance of greater than 12 inches while employees utilized the guardrail systems to access modular home roofs to conduct decking and roofing operations.
