29 CFR 1926.1204(c)(2): The employer did not provide each authorized entrant or that employee's authorized representative with the opportunity to observe any monitoring or testing of permit spaces; On or about July 8, 2024, at job site located at 9220 Old Santa Fe Road, in Kansas City, MO, the employer failed to implement permit space atmospheric monitoring or testing procedures. The company had a 2-person laborer crew enter active sewer systems via manholes during preparatory work for invert installation activities at a new apartment complex.
