29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. On or about August 23, 2023, an employee was exposed to a fall of approximately 13 feet while performing masonry activities on a residential home located at 2761 Torry Pine Drive, Medina, Ohio 44256. The employee was working on the upper level of the scaffold and the employer did not ensure he was trained in how to recognize and minimize fall hazards.
