29 CFR 1910.146(i)(6): The employer did not ensure an attendant monitored activities inside and outside a permit confined space to ensure it was safe for entrants to remain in the space and order the authorized entrants to evacuate the permit space immediately under hazardous conditions: a. Carestream Health, Inc., 2000 Howard Smith Ave West, C-40 bldg. Windsor CO 80550: On October 5, 2022 the employer did not ensure that an attendant was monitored activities inside and outside a permit required confined space to ensure it was safe for entrants while working in permit confined spaces. This condition exposed employees to delayed rescue in the event of an injury and burns.
