29 CFR 1910.146(d)(2): The employer's permit-required confined space program did not identify and evaluate the hazards of each permit space before employees entered them: (a) On or about April 28, 2022, and at times prior, at Milton R. Young Station Minnkota Power Cooperative 3401 24th Street SW, Center, ND where employees were allowed to enter a confined space, and were exposed to hazards to include fly ash that had accumulated to depths of three to four feet in areas of the Plenum with thermal burn, and known skin irritation and inhalation hazards.
