29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a) 159 Lexi Lane, Baldwinsville, NY 13027, on or about 1/11/22: Employees were inside a trench, installing the connection from the house to the main, 8 feet below grade, where a portion of that trench, closest to the house was cut vertically, spoil was placed to the edge of the cut, and where workers installed piping without a protective system.
