29 CFR 1910.244(a)(2)(vi)(a): Each jack used constantly or intermittently at a single locality was not thoroughly inspected at least once every six months: a) Vehicle maintenance: On or about November 6, 2021, the Pittsburgh 20 Ton Air Hydraulic Bottle Jack used to support the 2006 Chevrolet Express van had not been inspected, exposing employees to a crush-by hazard. b) Vehicle maintenance: On or about November 6, 2021, the unidentified hydraulic floor jack used to support the 2006 Chevrolet Express transmission had not been inspected, exposing employees to a crushed-by hazard.
