Hook-on and/or attachable ladders were not positioned so that their bottom rung would not be more than 24 inches (61 cm) above the scaffold supporting level: On or about March 05, 2021, on the northwest side of the project site, employees were performing masonry work on the outside of a building utilizing scaffolding with a ladder whos bottom rung was twenty-eight (28) inches above the scaffold supporting level.
