29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model, or make) was used, and at least annually thereafter: a) At the facility, 1621 Route 22 West Operating Company, LLC, Bound Brook, NJ. The employer failed to fit test employees prior to the required use of N95 respirators. The employer provided and required employees including, but not limited to, Registered Nurses, Licensed Practical Nurses, Certified Nursing Assistants, and Housekeeping staff to wear N95 respirators and respirators labeled KN95 at the facility. The employees provided direct care and services to suspected and confirmed positive COVID-19 positive patients. Violation occurred on or about 5/09/20, and thereafter. ABATEMENT DOCUMENTATION REQUIRED
