29 CFR 1910.134(k)(3): Training was not provided prior to requiring employees to use a respirator in the workplace: Heritage Care Center of Berkeley located at 4401 North Hanley Road, Berkeley, MO 63134: On or about August 10, 2020, the employer did not provide employees who were required to wear N95 respirators to protect against exposure to the SARS-CoV-2 virus with effective training in the use, cleaning, and storage of respirators, before requiring their use.
