29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: a) PB-15 Svartek Grinder: On or about April 7, 2020 and at times prior thereto the training provided by the employer for the recognition and control of hazardous energy was not complete or effective in that equipment and maintenance tasks involving hazardous energy were not identified by the employer when developing and implementing the program, or by authorized employees when performing the tasks. Employees and the employer did not recognize all of the types and forms of hazardous energy, such as the pneumatic pressure on the PB-15 grinder shovel cylinder guide rods, and thermal energy on the cooker steam header, as hazardous energy that needed to be included in the specific program procedures, training, periodic review and evaluation.
