29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings were not provided with covers identified for the purpose. If metal covers are used, they were not grounded. In completed installations, each outlet box did not have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass were not provided with bushings designed for the purpose or did not have smooth, well-rounded surfaces on which the cords may bear: a) On or about August 9, 2019, in the captive barge's break shack, the employees are exposed to electrical shock and the employer did not ensure faceplates were installed on three (3) outlets.
