29 CFR 1928.57(a)(6)(iii): Instruction was not provided to stop engine, disconnect the power source, and wait for all machine movement to stop before servicing, adjusting, cleaning, or unclogging the equipment. a) Facility: On October 2, 2108 and at times prior thereto the energy control training provided by the employer was not clear or consistent in identifying when an energy control device is needed when performing servicing and maintenance activities to include cleaning and clearing jams. b) Facility: On October 2, 2108 and at times prior thereto the energy control training provided by the employer did not clearly identify the roles and responsibilities between mechanics and mechanics helpers.
