29 CFR 1910.184(c)(6): Slings were not securely attached to their loads: The employer is failing to protect employees from struck-by and crushing hazards. This was most recently documented at 1351 S. Reca Ct, Wichita, Kansas, where employees were using slings to aid in the removal and replacement of the right and left Boom Hoist Cylinders on a Lull Highlander (Model 1044C-54). The employees were exposed in that the slings were not securely attached to their load. a) On or about 07/27/18, a Nissan forklift (CPH02A20V) was used to facilitate the replacement of the right Boom Hoist Cylinder on the Lull. In order to accomplish this task 8 foot slings were added to the forks using Countyline hitch pins (part#18PCH001TSC). The slings and pins were added to cradle the load. No additional means were provided to positively attach the slings to the load. b) On or about 08/03/18, a Toyota forklift (42-6FGU25) was used to facilitate the replacement of the left Boom Hoist Cylinder on the Lull. In order to accomplish this task 8 foot slings were added to the forks using Countyline hitch pins (part#18PCH001TSC). The slings and pins were added to cradle the load. No additional means were provided to positively attach the slings to the load.
