29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: Extrusion department, portable unknown/unmarked sheet shear: On or about January 2, 2018, hazardous energy control (lockout/tagout) authorized employee training was not provided to machine operators prior to being assigned to perform regular setup tasks (servicing) in the unguarded point-of-operation. Operators had their fingers and hands exposed to unexpected cutter start-up hazard likely to result in amputation injury when accessing the shear point to pull material through. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
