29 CFR 1910.1200(g)(2): The chemical manufacturer or importer preparing the safety data sheet did not ensure that it included the required headings and information in 29 CFR 1910.1200(g)(2)(i) through 29 CFR 1910.1200(g)(2)(xii): a) 331 Maple Ave, Hanover, PA - The SDS for Reynco 152-94M in Section 1, Identification did not list the address of the company creating the product, instead it listed the address of the company preparing the safety data sheet (SDS), on or about April 15, 2017. b) 331 Maple Ave, Hanover, PA - The SDS for Reynco 152-94M did not properly identify Section 2 as Hazard Identification, on or about April 15, 2017. c) 331 Maple Ave, Hanover, PA - The SDS for Reynco 152-94M did not have the proper classification in Section 2, on or about April 15, 2017. d) 331 Maple Ave, Hanover, PA - The SDS for Reynco 152-94, did not use the signal word, hazard statements, precautionary statements in accordance with 1910.1200, on or about April 15, 2017. e) 331 Maple Ave, Hanover, PA - The SDS for Reynco 152-94 in Section 3 did not have the proper identification as Composition/information on Ingredients, on or about April 15, 2017. f) 331 Maple Ave, Hanover, PA - The SDS for Reynco 152-94 in Section 3 improperly identified the product as non-hazardous and therefore did not list the ingredients as required, on or about April 15, 2017. h) 331 Maple Ave, Hanover PA - The SDS for Reynco 152-94 in Section 8 did not list the hazards of the ingredients due to the improper classification, on or about April 15, 2017. i) 331 Maple Ave, Hanover, PA - The SDS for Reynco 152-94 in Section 10, did not list all the required subheadings: "conditions to avoid", "reactivity" and "the possibility of hazardous reactions" on or about April 15, 2017. j) 331 Maple Ave, Hanover, PA - The SDS for Reynco 152-94 in Section 11, did not list all the required subheadings: "symptoms related to the physical, chemical and toxicological characteristics," "delayed and immediate effects and also chronic effects from short- and long-term exposure," "numerical measures of toxicity" and "the national toxicology program or International Agency for Research on Cancer" on or about April 15, 2017 k) 331 Maple Ave, Hanover, PA - The SDS for Reynco 152-94 did not include Section 16, "Other information" on or about April 15, 2017 l) 331 Maple Ave, Hanover, PA - The SDS for Reynco 152-94, improperly located the date of preparation as it did not include section 16, on or about April 15, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
