29 CFR 1910.213(b)(3): Provisions were not made to prevent woodworking machine(s) from automatically restarting upon restoration of power after power failure: a) A & D Wood Products, Inc., Elida, Ohio: On or about September 5, 2014, the employer did not ensure provisions were made to prevent the Irving Machine Works manufactured cut-off saw from automatically restarting when power was restored to the equipment. Employees used a circuit breaker located in a different room to power the saw on and off, exposing employees to laceration and amputation hazards in the event the saw was powered on unexpectedly.
