29 CFR 1926.1053(a)(1)(ii): Non-self-supporting portable ladder(s) were not capable of supporting without failure, at least four times the maximum intended load: Location: Jobsite An aluminum type II extension ladder, was overloaded and failed, causing the ladder jack scaffolding that it was supporting to fall and resulting in employee injuries when they fell from the scaffold to the ground below.
