29 CFR 1910.119(f)(1)(ii): The employer's written operating procedures did not provide clear instructions for safely conducting activities involved in each covered process consistent with the process safety information in that operating limits were not adequately addressed: In the Olefins Unit at Formosa Plastics Corporation located in Point Comfort, Texas, the employer did not specify the safe operating limits (permit limits) for reactor opening conditions in the Reactor Cleaning procedure and did not include the consequences of deviations and steps required to correct or avoid deviations.
