29 CFR 1910.146(c)(5)(ii)(C): Before an employee entered a confined space, the internal atmosphere was not tested with a calibrated direct-reading instrument for oxygen, flammable gases and vapors and potential toxic air contaminants: On or about May 27, 2014 and times prior there to, the employer did not ensure that the internal atmosphere of a tanker trailer was tested with a calibrated direct-reading instrument for oxygen content, flammable gases and vapors and potential toxic air contaminants: before an employee entered the confine space; there by exposing the employee to a hazardous atmosphere.
