29 CFR 1926.21(b)(1): The employer did not initiate or maintain an accident prevention program(s) as may be necessary to comply with this part: A.) Barry Smith Slate Roofing, Inc., PA Route 5 & Dutch Road, Fairview, PA 16415, on or about July 2, 2013 and times prior thereto: Employees who were utilizing electrical extension cords and electrical power tools. Employees did not have electrical safety training. B.) Barry Smith Slate Roofing, Inc., PA Route 5 & Dutch Road, Fairview, PA 16415, on or about July 2, 2013 and times prior thereto: Employees did not have personal protective equipment (PPE) training covering topics, including, but not limited to when PPE is necessary; what PPE is necessary; how to don, doff, adjust and wear PPE; limitations of PPE; and the proper care, maintenance, useful life and disposal of the PPE.
