29 CFR 1910.146(d)(3)(iv): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit entry operations. The employer does not implement the means, procedures, and practices necessary for safe permit entry operations to eliminate or control atmospheric hazards. This violation was occurred on or about June 14, 2013, and at times prior thereto, in the Methanol Convertor Reactor 7001 where employees were exposed to an atmospheric hazard while working in the reactor without developing and implementing the means, procedures, and practices necessary for safe permit entry operations. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that permit spaces are purged, inerted, flushed, or ventilated as necessary to eliminate or control atmospheric hazards.
