29 CFR 1926.955(c)(2): Prior to stringing operations a briefing was not held setting forth the plan of operation and specifying the type of equipment to be used, grounding devices and procedures to be followed, crossover methods to be employed, and the clearance authorization required: a. On or about March 20, 2013, on the west side of Hwy 2300, prior to employees removing conductors, a briefing was not held that covered all of the special precautions, and procedures to be followed, including but not limited to warnings and instructions on equal potential zones and induced voltages.
