29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: The employer fails to require adequate inspections of fall protection equipment for damage from weld burns and removal of defective components from service. This violation was most recently observed at MCAS Miramar Hangar 7. a) Protecta harness model no. AC17550 - On or about 11/1/2012, the employer failed to inspect and remove from service a fall protection harness worn by an ironworker having burn damage. b) Guardian harness model no. 01703 - On or about 11/1/2012, the employer failed to inspect and remove from service a fall protection harness worn by an ironworker foreman having burn damage.
