Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Employees working with, and in the vicinity of, the #162001 Underhung Crane were exposed to the hazard of being struck by loads falling from the crane in that the crane's upper travel hoist limit switch was not being checked for proper operation at the beginning of each work shift, and the switch was being used as an operating control in the normal operation of the crane. Among others, feasible and acceptable means to correct this hazard would be to test the function of the upper travel hoist limit switch before the beginning of each shift, in accordance with ANSI / ASME B30.11 the American National Standard / American Society of Mechanical Engineers standard for Monorails and Underhung Cranes, and to refrain from using the switch as an operating control, also in accordance with the standard.
