Text For Citation: 01 Item/Group: 001C Hazard: EXPLOSION
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire, explosion and steam hazards: (a) Boiler room; Preventative maintenance inspections were not conducted on a frequent and periodic basis nor were permanent records maintained on the three "Titusville" automatically fired natural gas boilers that were converted from oil fired. Interchanged equipment and parts, repaired combustion chambers, adjusted air/fuel gas ratios for the main and pilot gas valves, altered control panels, a clogged drip leg and a partially opened hydramotor valve were on the boilers. Among other methods, one feasible and acceptable means of abatement is to initiate a progressive and permanent record system for modifications, alterations, materials, parts, and repairs as outlined in accordance with API 510, ASME CSD-1-1995 and the National Boiler Inspection Code, ANSI NB-23 .
