29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures: a) Caustic Department, 15696 Energy Road, Proctor, WV 26055: On or about May 11, 2025, contract employees performed servicing such as tightening bolts on the upper manway cover of the 1A surface condenser that was under an internal pressure of approximately 58 psi, exposing employees to a potential struck by hazard. The employer did not ensure the outside employer was informed of the lockout procedures and hazards of performing tasks under pressurized vessels exposing employees to potentially harmful energy. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.
