29 CFR 1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section. Location: Worksite On or about March 11, 2025, employees were exposed to fall hazards of over 20 feet while performing siding activities while working from unguarded pump jack scaffolding, where the employer did not ensure that fall protection was in place. The employer has previously failed to ensure that fall protection was in place as seen below in prior OSHA inspections: a) Lifetime Contractors Inc. was previously cited for a Repeat- Serious violation of this occupational safety and health standard, or equivalent standard, 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1785846, citation number 1, item number 1a and was affirmed as a final order on 12/2/2024, with respect to a workplace located at 26 Union Street, Natick, MA. b) Lifetime Contractors Inc. was previously cited for a Serious violation of this occupational safety and health standard, or equivalent standard, 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1595087, citation number 1, item number 1 and was affirmed as a final order on 11/25/2022, with respect to a workplace located at 133 Boardman Rd, East Boston, MA. c) Lifetime Contractors, Inc. was previously cited for a Serious violation of this occupational safety and health standard, or equivalent standard, 29 CFR 1926.501(b)(4)(i) which was contained in OSHA inspection 1247789, citation 1, item 1 and was affirmed as a final order on 11/9/2017, with respect to a workplace located at 84 Obery Street, Plymouth, MA 02360, where an employee was seriously injured after falling approximately eleven (11) feet to the ground below from a floor hole opening that was not protected.
