29 CFR 1910.119(e)(3)(iii):The process hazard analysis did not address engineering and administrative controls applicable to the hazards and their interrelationships such as appropriate application of detection methodologies to provide early warning of releases: Cargill, Inc. DBA Cargill Meat Solutions Corporation, 1505 East Burlington Avenue Fort Morgan, CO 80701: On or about February 10, 2025, employees were exposed to inhalation hazards associated with ammonia from the following conditions: (a) The process risk assessment (PRA) did not address the improper seating of a Henry Technology Pressure Relief Valve (serial number 1422, category 5602, size ? inch, set PSI 250, capacity 57.7) on the sub-roof containing the liquid header. (b) The process risk assessment (PRA) did not address the improper seating of a Henry Technology Pressure Relief Valve (serial number 1419, category 5602, size ? inch, set PSI 250, capacity 57.6) on the sub-roof containing the liquid header.
