29 CFR 1910.146(d)(5)(ii): The employer did not test or monitor the permit space as necessary to determine if acceptable entry conditions are being maintained during the course of entry operations (a) 402 Tatamy Rd., Nazareth, PA - Dryer/Kiln: On or about January 23, 2025, employees engaged in welding, torching and grinding inside the dryer/kiln were exposed to entrapment and hazardous atmospheric conditions as a result of the employer's failure to test or monitor prior to entry.
