29 CFR 1926.50(b): Provisions were not made prior to commencement of the project for prompt medical attention in case of serious injury: At a worksite located at or near 13667 N Kentwood Lane, Neosho, MO 64850: Workers were exposed to struck-by and crushed-by hazards from earthmoving equipment causing a serious injury. This was demonstrated on or about December 20, 2024, the employer failed to ensure that employees knew how to effectively contact medical services in the event of an emergency. A five-person crew performed utility installation services with heavy equipment that included a Ditch Witch directional drill and Bobcat E35i mini excavator. Two employees were crushed-by the excavator. The uninjured employees did know how to summon emergency services and relied on nearby homeowners to call for assistance.
