On a scaffold systems, at a temporary worksite located, at or near, the southside of 1090 Bella Vista Boulevard in Saint Johns, Florida 32084: On or about September 10, 2024, the employer exposed employees to fall hazards ranging from 10 to 15-foot, in that guardrail systems were not installed along all open sides and ends of platforms as employees performed masonry work.
