29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a) On July 17, 2024, at the above address, employees were exposed to cave-in hazards while performing work in an unprotected excavation of approximately five feet and eight inches deep. The employer did not ensure that employees were protected from cave-in hazards. No abatement certification or documentation is required for this item. Mango Construction, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1) which was contained in OSHA inspection number 1509682, citation 2, item 2, and was affirmed as a final order on July 6, 2021, with respect to a worksite located at 104 North Chester Avenue in Park Ridge, Illinois. Mango Construction, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1) which was contained in OSHA inspection number 1431367, citation 2, item 2, and was affirmed as a final order on July 17, 2020, with respect to a worksite located at 1101 Green Bay Road, Highland Park, Illinois.
