29 CFR 1926.451(f)(1): Scaffold and/or scaffold components were loaded in excess of their maximum intended loads or rated capacities, whichever was less: a) On or about June 6, 2024, at the above addressed job site, components of the Self-Riser Climbing Formwork System used by employees as a temporary elevated platform were loaded in excess of their maximum intended loads or rated capacities, thereby exposing employees to hazards associated with falls.
